     -PURPOSE STATEMENT-

Jesus said…“ I was sick and you looked after me…” Matthew 25:35,36 

This free clinic is centered on the belief that citizens who work at being productive should not be denied ordinary medical care while making their own effort to provide for themselves and their families.


The Mission of the Mustard Tree Wellness Clinic is:

  • To show God’s love through community wellness care.
  • To operate entirely on private donations and volunteer services.

 

The following is an outline of the steps to be taken in order to qualify as a patient of the Mustard Tree Wellness Clinic:

The Clinic was established to serve the “working under-insured” of this community, as outlined in our Purpose, Vision, and Mission statements. It is not an emergency clinic.

The criteria for acceptance as a clinic patient is that:

  1. The prospective patient has little or no health care insurance, Medicaid or Medicare coverage.
  2. Inadequate financial resources.
  3. He/She is employed or actively seeking employment. Other difficult life situations may be considered.
  4. If the prospective patient is deemed eligible, arrangements will be made for their medical care.

  5. Once accepted the patient will receive free ordinary medical care by a volunteer physician.

  6. The Clinic is open Monday through Thursday, 9:00 am–5:00 p.m.  and Tuesday & Thursday, 5:45–7:00 pm.  Please call for an appointment.  Closed Fridays and Holidays.

  7. The prospective patient should call the Clinic Office line at 734-2610, 9:00 a.m.-5:00 p.m., Monday through Thursday, for an interview to determine their eligibility.
